Witnessing of signature on affidavit for use in Singapore

Procedures

1. Please contact us and schedule for an appointment (singemb_was@mfa.sg). All applications will only be accepted between 9.00am to 1.00pm on weekdays on an appointment-only basis.

2. As we have to prepare certificates in advance, please provide the following:

   • Number of people signing on the document

   • Name(s) as it appears on the passport

   • NRIC number(s) or Passport No(s)

   • Gender / Date of Birth

   • If not a Singapore national, please indicate nationality

   • Number of documents for signature attestation

   • Scanned copy of documents

3. On the day itself, please bring the following:

   • Original passport for ID and signature verification

   • Photocopy of passport page showing name, date of birth, IC / passport number, for our retention

Processing fee

• US$ 3.00 on taking or re-taking an affidavit or a declaration for each person making the affidavit or declaration

• US$ 1.00 for each exhibit referred to in an affidavit and required to be marked

• All fees are subject to change without notice.

   All applications submitted in person to the Singapore Embassy in Washington DC will only be accepted between 9.00am to 1.00pm on weekdays on an appointment-only basis.

   1. Payment can be made by cash. Alternatively, please issue a cashier’s cheque or money order in US currency made payable to the "Singapore Embassy". Personal/corporate checks are not accepted.
      

2. Singapore Consulate-General in San Francisco

   1. Taking an affidavit can only be done by the Honorary Consul-General in person. Please call ahead to make an appointment.

   2. Payment can be made by cash. Alternatively, please issue a cashier’s cheque or money order in US currency made payable to the "The Consulate of Singapore". Personal/corporate checks are not accepted.
